Why am I unable to get a match on nurses for my entity provider list?

Health care entities can access and download a Provider Bridge Passport for nurses that have opted-in through Nursys e-Notify.  Nurses added to an entity  provider list who have not opted-in will be identified by a !  located to the left of the nurse's name.  The Provider Bridge Passport is only available for nurses who have opted-in.
